What Is Forehead Reduction Surgery?
Forehead reduction is a precise, structural procedure that lowers a high hairline and shortens the vertical height of the forehead. Unlike hair transplantation, which relies on new transferring hair follicles to promote new hair growth over time and potentially multiple rounds, hairline advancement provides immediate results.
Hairline lowering surgery works by advancing the hair-bearing scalp forward and removing some of the excess forehead skin. Most patients in the can achieve 1.5–2.5 centimeters of lowering in a single procedure, depending on scalp mobility and individual anatomy. This allows one to have better balance their facial proportions, bringing the upper face more in line with the heights of the mid and lower face.
Who Is a Good Candidate for Forehead Reduction Surgery?
Ideal candidates typically:
- Are bothered by their naturally high hairline, resulting in a “long” or “big” forehead
- Possess good scalp laxity
- Have stable hair density
- Have lower set eyebrows or are also looking for brow elevation
Careful evaluation is essential to ensure the results are safe, natural-looking, and harmonious with the rest of the face.
How Is the Surgery Performed?
The procedure is performed in an accredited surgical facility under general anesthesia or local anesthesia with sedation.
The new hairline is designed to follow the natural contours of your scalp. Instead of a straight line, a slightly irregular, trichophytic incision is used, allowing hair to grow through the scar for a discreet result, while hidden within the newly positioned and created hairline.
The hair-bearing scalp is released from its attachments to the skull from the hairline all the way posteriorly to the back of your head. This allows the hair-bearing scalp to be advanced as far forward as possible. This surgery is done while preserving the nerves that provide sensation to the scalp.
With the hair-bearing scalp fully mobilized and pulled as far forward as possible, the amount of forehead skin that can be excised is marked. This excision is done very carefully to balance the desired amount of forehead skin reduction to shorten the forehead while not creating too much tension that would otherwise result in a widened scar.
Deep, internal-fixation sutures into the skull are then placed to advance the hair-bearing scalp forward. These internal sutures anchor the advanced scalp to the skull, redistributing tension away from the skin incision and helping ensure a long-lasting, natural hairline.
After fixation, the incision is closed in multiple layers. The trichophytic closure encourages hair growth through the scar, helping to camouflage it naturally. With proper surgical planning and tension control, the incision line typically becomes very difficult to detect once hair regrows, even with hairstyles that pull the hair back.
Considering Hairline Advancement in Virginia or Maryland?
The first step is a personalized consultation. Forehead reduction is a structural and aesthetic procedure, and my approach is data-driven and patient-centered. Experience matters. A successful forehead reduction relies on accureate assessment of facial proportions and scalp mobility, careful incision design, deep fixation sutures, and meticulous suturing. Together, we can design a plan that achieves a natural, balanced hairline and long-term results.
